Use this form to request a Good Standing letter for ministry in any diocese in the United States. Please complete the following form in its entirety and click submit. To process your request, please allow at least two weeks (three weeks during June through August) prior to your departure. Please note that incomplete Request Forms will not be processed. Please complete a separate request for each Diocese you will be visiting.
